Farm to Institution Guide Submission Form


Please read the criteria for inclusion in the guide. Farm to Institution staff will review each submission and contact you for any additional information we may need.  

Criteria:

  • Farmers must have the capacity to serve, and understand the needs of the institutional marketplace.
  • Farmers who do not distribute their own products must have a proven method of delivery to institutions.
  • Local Farmer Distributors and Cooperatives who supply products from more than one farm must preserve each farm’s identity throughout the supply chain, e.g. farm name on boxes and/or packing slips.

Please fill out the submission form carefully. If you are not familiar with selling to institutions we strongly recommend you consult with the Farm to Institution Project Manager
